Basically, when I unplug the modem (usually accidentally), linux will often freeze....

Yea, I think we're pretty much hosed. I don't think any of the maintainers have ever paired a bluetooth device with bt5 :-\
I solved #1 by deleting /etc/udev/rules.d/70-persistent-net.rules
